1Pharmacy benefit must be ordered from the plan formulary
by a plan-affiliated physician. If the beneficiary obtains routine care
from out-of-plan providers, neither Medicare nor Elderplan will be responsible
for the cost of care. Co-payments may apply and vary based on use
of in- or out-of-network pharmacies and your yearly out-of-pocket drug
costs. The formulary contents are subject to change within a contract
year without advance notice. Please contact Elderplan for additional details.
2All routine care must be provided by plan providers,
except in emergency situations and for out-of-town renal dialysis.
If you obtain routine care from out-of-plan providers, neither Medicare
nor Elderplan will be responsible for the costs.

Elderplan is a Medicare Advantage Special Needs Plan operating under
a Medicare Advantage contract. Anyone entitled to Medicare Parts
A and B living in Brooklyn, Manhattan, Queens, Staten Island or the
Bronx may apply. Enrolled members must continue to pay their Medicare
premiums. This plan is available to anyone who has both Medical Assistance
from the state and Medicare.
